Please note that all Appeals must be received in writing 21 days of the Notice of Claim date to be eligible for review.

Please send your written appeal, on the downloadable PDF below and fill in all the field including your Notice of Claim Number, Reason for Appeal and return postal and email address to the address below:

Appeal Guidelines
Appeals can only be made in writing and must be received within 21 days of the Notice of Claim date to be eligible for review.
For all appeals, make sure that you include your;
Notice of Claim Number
Reason for Appeal
A way to contact you – name, postal address and email address
Once we receive your appeal, the Notice of Claim will be placed on hold until we can review and respond. If we need more information from you, we will respond in writing.
We will confirm in writing if your appeal has been successful, or if the notice is upheld and you are still required to pay the notice.
Please note that the fastest way to make an appeal and to receive confirmation is via the online appeal portal.
